Sick Leave and Fit Notes
If you require a sick note you should phone the surgery and the Advanced Nurse Practitioner will be able to discuss this with you. An appointment to see a clinician may be necessary.

Some administration tasks can be requested via E-Consult which is available via the Home page
A copy of your sick note can be emailed to you or attached to a text message for you to forward to your employer digitally. In this case the original will remain in the surgery for you to collect when able to do so.

Specimens
Please only use appropriate specimen containers for any sample, these are available at reception.
Ensure your sample is received in the surgery before 1pm on normal opening days to allow the hospital collection service to transport your sample to the lab without undue delay. Alternatively, if you have the necessary paperwork, you can deliver the sample yourself to the Path Lab reception at Wrexham Maelor Hospital.
For this reason it is best to book your blood test in a morning appointment.
You can make an appointment to have a blood test at Wrexham Path Lab or Grove Road Phlebotomy service by calling 03000850042 you will need to have the request form which can be obtained either from the surgery or you can use ones issued at the hospital. Once you have your appointment please remember to take your form or they won’t be able to take the sample.
